Financial Policies

Payment for all services is due at the time services are rendered. However our office will submit a claim for you to any insurance carrier with whom Dr. Ziyad Mugharbil contracts.

Any fees not paid by the insurance carrier are the patient’s responsibility.

Dr Ziyad Mugharbil accepts most major insurance plans. including employer-based plans, Federal Health Insurance Marketplace plans as well as traditional Medicare and Medicare Advantage plans.

All new patients are required to read and sign our Polcies and Payment policy that explains our billing practices. Patients without insurance are required to pay their fees at the time services are rendered. If payment plan is needed, a financial hardship form will be accepted to determine payment.
